This is a very interesting article. This is also the first time that I have actually read about such problems as “off-peak” hours, though I have been one to experience it. It is a topic that is necessary to talk about and definitely try to find solutions to problems that are generated during these hours for the betterment of care for the patient.

I too, did not realize the majority of nursing hours were off-peak. I did not really think of weekend days as off peak, but there is limited access to ancillary staff, managers, and physicians during this time. I agree with Zimmerman that a “need to look for a comprehensive picture rather than focus on one thing” needs to be considered when looking for a solution to off-peak mortality. Can the lax environment be a cause? No managers, administrators, and a decrease in physician presence so the staff maybe less by the book?
Thank you for this informative article. This is something that we always look at when reviewing incidents. We strive to make sure our patients have the same care whether it be 3am or 3pm. Lots of great information and interesting thoughts to think about.
